A Renton doctor discovered two prescriptions for Oxycodone had been forged using his name, when pharmacies requested a confirmation of the prescription, Aug. 24.
The doctor, located on the 100 block of Park Avenue North, had once before been a victim of identity theft.
He knew the prescriptions were forged, because the format was incorrect, his name was misspelled and his signature incorrect.
However, both the forgeries had the correct Drug Enforcement Administration number.
